This toolkit provides educational materials and practical ideas for how you can bring awareness and conversations about the end of life to your LGBTQ+ community all year long.

A vital part of advance care planning is creating an advance directive, a legal document that describes your goals for your healthcare, who you want to be involved in your care, and who will represent you if needed.

This form enables people not traditionally recognized as family members to gain priority visitation rights. Once completed and signed, it should be kept with your advance directive.
